Anonymous feedback from students to the department

If you have taken or are taking a course in our department and have comments or suggestions about our courses and programs, you are invited to use this Qualtrics survey to transmit them to us securely and completely anonymously. The outreach committee of the mathematics department will regularly read your comments and bring them to the attention of the appropriate people and committees within the department. 

Please do not use the Qualtrics survey to report discrimination or misconduct. Instead, please visit Reporting Discrimination. 

Our responses, and changes made in response to your comments, will be posted as responses to feedback from students to the department. We do not guarantee that we will follow each suggestion, but we do guarantee a thoughtful reading of every comment.
